I’ve been looking at this one for awhile, when it finally hit me! This must be from the very beginning of “Hot Lips is Back in Town”, when the staff meeting is being held.
When I watch the episodes on my computer, I usually just jump over the opening, so I probably missed this very image. That just has to be why I didn’t recognize this one.
