This was a really interesting episode when Sidney, I think it was a good move on the writers' part to show what would happen if a patient actually turned on Sidney; throughout the series, Sidney had this theory that there's a better chance at helping a patient by sending them back to the front as quickly as possible, otherwise, coddling them, or sending them stateside only deepens their guilt... but we see the method didn't work with one patient, who swears that he hates Sidney's guts for sending him back to the front, letting him get wounded.
The bonfire thing offered a visual treat for everyone, they burned just about everything in camp, short of the camp itself (even Potter's desk, Sidney's fatigues... and Charles's cot).
But why in the world was Igor played by some random other guy?

The method did work. The patient became much more fully functioning. So he hated his doctor... that's not as important as keeping him from being a nonfunctioning victim of combat fatigue for years or possibly his entire life.
