Well, it would have to be an officer's rank, because they used it to get Radar into the Officer's Club. I had thought of it as between 1st Lieutenant and Captain, but I don't think it actually said in the episode.

The method did work. The patient became much more fully functioning. So he hated his doctor... that's not as important as keeping him from being a nonfunctioning victim of combat fatigue for years or possibly his entire life.

After Harris made a fool of herself with Father Mulcahy, I don't think she was about to fall for anyone else at MASH. And I didn't see Hawkeye being really smitten with her. To him, she was just a female, and he did the reflexive over-the-top flirting he did with all the new females.
